Other top rated fertility clinics boost their Live Birth numbers by inserting multiple embryos, often resulting in twins or triplets. At ORH we will only implant one healthy embryo per cycle for a safe pregnancy. Because our embryos are frozen, they are ready when you are for baby number two. We won't risk the health of our moms by inserting multiple embryos to inflate our success rates.

Also, we won't push patients prematurely into using donor eggs to increase our numbers. If a patient's egg quality is marginal or they know they want more than one biologically related child, we'll counsel options for multiple retrieval cycles to obtain a genetically viable embryo or embryos for implantation.

While SART would have us emphasize the statistic of Live Birth/Intended Retrieval, we believe this disincentivizes clinics from doing all the things we believe in such as allowing patients to cycle using their own eggs rather than donor eggs – even if their prognosis is poor, allowing older patients to do more than one retrieval to accumulate viable embryos for more than one child, family balancing, universal PGT-A and SET (Single Embryo Transfer). We believe the most important metrics a patient should use to guide their selection are: number of embryos transferred, implantation rate, and live birth rate per transfer.

RFID Tagging of All Patient Samples (IVF & IUI)

We understand the immense level of trust that patients put in their fertility clinic to safeguard the identity of their most precious samples during reproductive procedures. To that end, we added another layer of security through the use of radio frequency identification (RFID) tags. Not only were we the first clinic in the USA to adopt the use of RFID tags in all of our ART (IVF and IUI) procedures, we remain one of only a few clinics in the country with such a system.

RFID tags provide an additional level of oversight by constantly monitoring the identity of the samples throughout the process. The use of this system has provided both our patients and staff with peace of mind.

Freeze-All IVF Protocols

Our new standard in IVF treatment

We have adopted a new standard in IVF: Routine day 5-7 Preimplantation Genetic Testing for Aneuploidies (PGT-A), a Freeze-all vitrification protocol, and Single embryo transfer (SET). Adopting these practices into our routine care have resulted in dramatic increases in the success rates at our practice.

The proof is in our numbers. In the last complete reporting year, our implantation rate reached a staggering 86.4% in patients under the age of 35! That means 86.4% of the transfers we performed in 2018 resulted in a pregnancy. Now, not every early pregnancy (implantation) results in a live birth, just as not every retrieval will result in a viable embryo to transfer. Biology is just not that simple.

According to Genesis Genetics and a recent abstract(1,2), there are only a small number of U.S. clinics that are currently doing this routinely. A recent study reported a 97% increase in live birth rates using this protocol, without SET, vs the older practice of day 3 or day 5 fresh transfers. Although SET was not used throughout this study the chance of pregnancy per embryo transferred was more than double with PGT-A and freeze-all compared with cycles without PGT-A(3). It should be noted, however, that not all studies have shown this degree of impact.

Increasing IVF Success

Preimplantation Genetic Testing for Aneuploidies (PGT-A) is a test that can be performed as part of an IVF cycle. PGT-A works by looking at the genetic material of each embryo and making sure that the embryo is genetically normal and contains the correct number of chromosomes. Abnormal embryos generally do not lead to successful pregnancies and can result in the birth of a child with a genetic condition.

The technique of PGT-A requires IVF and growing embryos to the blastocyst stage (day 5 to 7). Many IVF labs are not equipped to carry out this level of test. The embryo must be “hatched” in the IVF lab, and a few cells biopsied from the outer trophectoderm layer of cells (which later goes on to form the placenta and membranes of a pregnancy). The embryo is then vitrified (an advanced technique of embryo freezing), and stored for later transfer back to the uterus. The biopsied cell samples are sent to a PGT-A lab for a count of chromosomes.

Embryos are stored after sampling, since genetic testing takes about a week. Embryos do well with the freezing-thawing process, with >97% of embryos surviving vitrification and thaw. Implantation rates improve too with a delayed transfer cycle as we’re able to bring the lining of your uterus to optimal conditions for carrying a healthy baby to term.

Identifying embryos with genetic abnormalities before they are transferred can have numerous benefits, including, but not limited to: a higher chance of pregnancy, and a reduced risk of miscarriage. We now utilize advanced next-generation PGT-A on all IVF patients.
